Quality Improvement Program: Subject Survey Tool

As part of your Self-Assessment you may wish to conduct a brief survey of the subjects enrolled in your study.  The Office of Research Integrity has prepared a survey to assist you in obtaining pertinent feedback from subjects. This survey should serve as a tool to help you assess the strengths and weaknesses of your informed consent process.

Please note, you should not treat a subject differently because of any outcome from the survey. The information you obtain should only be used to identify how the informed consent process can be improved.  Every effort should be made to prevent anyone who is not on the research team from knowing who responded to the survey and what responses were provided.
