International applicants now in the United States in non-immigrant status must complete the Required Supplemental Information and DSO/RO Report (pdf) and submit it to the Designated School Official/Responsible Officer of their current or most recent institution together with photocopies of all I-20/DS-2019 forms, visa stamps and the I-94 departure card.
