PHS 398 and 2590 Revisions
The National Institutes of Health has announced that effective
Jan. 10, 2002 use of the revised PHS 398 and 2590 forms
is required. Applicants
are urged to always check the PHS 398 and PHS 2590 Web sites
to download the most current versions of the instructions
and forms prior to submission of an application to NIH.
Applicants should discard previous versions of the
PHS 398 instructions and forms, and use the current version
to ensure they have the most accurate information. FAQs on the PHS 398 and 2590 are available at:

Zip Codes for the National Institutes of Health
The NIH, in response to security concerns, now
requires all United States Postal Service (USPS) mail
addressed to NIH to use the unique NIH zip code 20892.
All USPS mail addressed to the National Library of
Medicine should bear the unique NLM zip code of 20894.
All mail using 20892 and 20894 Zip Codes will be
cleared through the NIH North Stonestreet Mail Facility.
Special procedures and precautions will be used to screen
the mail before it is delivered to the various NIH offices
on and off campus. This
is an important measure to provide for the safety of all
individuals who must handle mail. The PHS 398 instructions have been updated (http://grants.nih.gov/grants/funding/phs398/phs398.html)
to reflect changes in applicable laws and NIH policies, and
to clarify instructions (see notable changes below.)
These updates incorporate a number of helpful
