RESEARCH SHARED EQUIPMENT MAINTENANCE PROGRAM REQUEST TO ADD EQUIPMENT

Equipment maintenance is provided through the Thermo-Fisher Scientific Co. and departments must request that equipment be added. Addition of equipment in categories 1, 2 & 3 is contingent upon availability of funds. Please follow the steps below to request that equipment be added to the program:

    1) Contact Sejla Marshall - (859)257-7128 and request a quote for the maintenance fee on your equipment.
    2) Complete the form below and attach the Thermo Cost Summary then hit submit:
    3) You will be notified whether your request has been approved, generally within 3 business days.

Category 09-10 Program Pays
1 50%
2 50%

4 0%

To justify the requested category for each piece of equipment:

  • For categories 1 and 2, name the Core Facility in which the equipment is located
  • For category 3, tell how many people from how many different departments use the equipment
  • For category 4, no justification is required

NOTE: The Maintenance Program does not support the following equipment with matching funds, however departments may add them as category 4 instruments: Centrifuges that are not ultra-centrifuges, computers, software, research lasers, spectrophotometers, freezers, and clean rooms.
